Mobile On-Site Mammography will come to Queen Creek from 1-6 p.m. Wednesday, Dec. 18 to offer mammography screenings.

Exams will take place at Queen Creek Town Hall, 22350 S. Ellsworth Road. Appointments are required; contact MOM at 480-967-3767 to schedule an appointment. Participants should have their insurance card ready when scheduling, according to a press release.

Participants need to have their insurance card; doctor’s name and address; and location of where they had their last mammogram films.

Most major insurance companies are accepted. Participants should consult with their individual plan for specific coverage and limitations, a release states. Participants will be responsible for any charges not covered. The cost without insurance is $217.

Baseline mammograms are recommended for individuals 35-39 (unless family history or symptomatic) and most insurance plans cover women every year 40 years and over.

Individuals under 35 must provide a doctor’s referral. MOM is able to accommodate patients with breast implants.
